What the Detailed Notice of Discharge Is
The Detailed Notice of Discharge is a form called CMS-10066. It comes from the hospital, after the "Important Message from Medicare" (IM) notice, CMS-10065. Both notices are tied to your Medicare discharge-appeal rights, under 42 CFR 405.1205–405.1206.
When the Hospital Must Give It to You
Once you call the Quality Improvement Organization (QIO) to ask for a fast appeal, the hospital must give you the Detailed Notice of Discharge in writing. This happens while the QIO's independent doctor reviews whether your parent is ready to leave.
What It Means While You Wait
- Medicare keeps paying during the QIO review.
- The hospital cannot bill for the days under review.
- The same QIO right applies to Medicare Advantage, not only original Medicare.

If You Still Disagree After the Detailed Notice
If the QIO's independent doctor rules against you, there is a second level: an expedited reconsideration. Ask the case manager about safe-discharge planning questions in the meantime.
